HealthCare Services

Picture
Global World Net E-Commerce (GWNE) has provided health care solutions to the health care community since 1991.
Our solutions range from design and implementation of Hipaa compliance solutions to HL7 application solutions. Some of our projects include the following:
Project: As an on-going project GWNE is presently building and maintaining an Electronic Health Record System. (EHR) The EHR / Physician Practice Mgmt System is triggered by the Event of a new patient coming to the Physician's office or If a new Patient is admitted or discharged from a Hospital. The system is  design is based on:

 1. select and enter new Patients
 2. Display Patient demographics, Height, weight, medical encounter dates display any special Patient allerts,
 3. List present and past encounter problems and add new problems
 4. List all present, past Allergies and add new Allergies.
5. Display past Physician notes and add new notes
6. Record and display vitals and record new readings
7. Display past Physician orders by date and Send new orders and receive response and Reports on orders
8. Interface with Lab Information Systen-LIS, Radiology Instrumentation via RIS, Nursing Station
instrumentation via NIS, Pharmacy Meds Dispenser via PHIS, Dietary Food Systems -DIS, Order Entry
    System,and accounting and billing services.

The following is a real world example of  a complex business scenario health care process that occurs when a patient enters a hospital, and a physician sends the patient for a lab test. Involved in this procedure are five parties:
  The attending physician
  The Hospital Registration System Tech
  The Clinical Order Entry System
  The Laboratory System
  The Billing System
The following steps could occur in this typical HL7 business process:

A person has some Physical or Neurological ailment or complaint. So, the person decides to see his/her General Practicianer, or go to the a Hospital ER. Upon arriving, reviews the persons complaints and symptoms
the attending hospital tech registers the patient, contact information is taken, insurance information is taken and the patients Medical Records are pulled or requested. Physician is assigned if the patient is not at his/her GP. While the patient is waiting in Triage, a Nurse or Tech vitals are taken and recorded. Bloodwork is taken if required. A list of Allergies, Diseases, and past medical encounters are reviewed. If upon examination by the  Physician the Patients complaints can be satisfied by outpatient treatment, the Physician will order the appropriate Medication, thearpy, diet, or lab testing and next appointment schedule and send the person home. Else, the Physician will Order that the patient be registered and admitted to the hospital. This will trigger a whole series of events. First an account will be opened for the In-Patient. A hospital bed in the appropriate ward in a specific bed will be assigned. Since a patient can be admitted for ICU, Observation, or Med/Lab/Surgery, the head of the specific department will be notified so he/she can assign a team of appropriate Physicians. The head Physician is notified via the ADT (Admition/Discharge Transfer) system. Other personnel such the Nursing Head, Labratory Head, Radiology Imaging, Dietary Head and Pharmacy Heads, Accounting & Billing, HouseKeeping and Instrumentation are all notified so as they can ready their staff for a new patient. In a typical modern hospital facility process starts by sending an ADT Electronic message as:
An ADT^O04 registration message is broadcast by the Hospital Registration System.
The ADT^O04 message is received by all departments that subscribe to the message, including the Clinical
Order Entry System and the Laboratory System.
The doctor orders a diagnostic test from the laboratory facility.
An ORM^O01 order message is sent from the Clinical Order Entry System, after validation of business rules.
The laboratory receives the order, and returns a confirmation.
An ORR^O02 order-confirmation message is sent by the Laboratory System, indicating that the order can  be executed.
The ORR^O02 message is received by the Clinical Order Entry System.
On completion of the tests, the laboratory sends the results to the doctor and other departments.
An ORU^R01 test-results message is sent from the Laboratory System.
The ORU^R01 message is received by the Clinical Order Entry System and the Billing System.
The Interface Engine sends out an e-mail message to the doctor, who receives the lab results on his wireless
PDA. (Or at his networked Desktop or laptop.
The laboratory receives the order, and returns a confirmation.
An ORR^O02 order-confirmation message is sent by the Laboratory System, indicating that the order can be    executed. Note: Ordrs are not just sent to Labs as in this example. Orders can be sent to any of the Team Stations like Dietary if the patient is a diabetic whoose sugar must be limited or a Heart patient whoose salt must me limited
Or a nursing station, whereby the patient vital must be taken every half hour.

Other HealthCare Projects we have been involved in include:
Project:  State DOH  Design and implement Medicare/Medicaid Hipaa 837I, P, D and 835 (and other) Transaction sets using Biztalk  2002 w/ the Biztalk accelerator. Migrated the BT 2002 design to BT 2004 w/ Biztalk. Accelerator .

Project: DOH Biztalk Hipaa  VB6 Patient billing system Conversion to VB.Net. Convert a stand alone patient billing application written in VB6To a VB.Net  application that interfaces with web services using Biztalk
Server 2002 as the interfacing  structure.

Project: Sub Contract for Medical Center Facility in Kansas City, MO. Used Biztalk 2002 Server and The MS Hipaa Accelerator for Biztalk 2002 to implement the 837 Transaction sets. I created the
Various Channels, Ports, Organizations, and  FlatFile to EDI Maps,
Xlang schedules, and AIC’s to send UB92 and Data Extract files to 837i Destination Organizations.
I addition we set up the Project servers and instructed the team in the Use of Biztalk. Also I researched and evaluated several  related software tools that were required for the project.

Project: Hipaa Standards Development and EAI System Implementation project. We created a training and education program to help Health Care Providers and Health Care Providers to implement the WPC recommended 5 point Preparation Process compliance. (Education/Training Awareness, Impact analysis, results of planning analysis, Implementation, Audit. The implementation part included using the MS BizTalk 2000 server To create a generic and flexible template of business workflow, which allows Testing for, proposed infrastructure changes. The model also allows XML data transport to EDI format and the reverse for legacy Partner Interfacing. Completed the process of modifying and or reproducing the Code for the .Net Platform and converting to VB7 (VB.Net/C#) and ASP.Net Respectively This project was expanded to provide MS .Net compact wireless device field interface to the web services on the web server. This will provide remote services to both Physician HealthCare support staff.
